How to Renew or Become an ASD Approved Vendor


Forms & Documents to Submit:

  1. Invitation to Submit Packet, submit this form with included documents below to ASD purchasing department:
    • Alaska Business License
    • W-9 Form
    • Background Check (I recommend going to the 5700 Tudor location as it requires no appt. More info on link)
    • Fingerprinting  (I recommend going to CasTech Fingerprinting as it requires no appt. & is also off Tudor Road. More info on link) 
    • Proof of Insurance (you submit your own general liability insurance certificate)

Then Finish Becoming an EAH Teacher:

  1. Then Submit EAH Teacher Agreement. (Due Feb. 1 every year).
  2. Await Official Acceptance from the Director
  3. Set up Google Classroom Account & Class/es
  4. Confirm Your Schedule with the Director
  5. Begin Teaching When Classes Start!
